At our April 2002 meeting, pharmacist Jim Enyart showed us how to keep track of our medical history on our computer. The convenience of taking this recorded history to your doctor saves him time and gives him valuable information that you might otherwise forget if you didn't have it all recorded on your computer. By keeping all this information recorded in one place, you're less likely to forget it years down the road when your doctor needs it. You can add to it as new medications are prescribed and as your health changes over the years. Each time you go to the doctor, just print out the newest version of your medical history. Then add to it after each visit so that it will stay current.INSTRUCTIONS FOR FILLING OUT THIS HEALTH RECORD FORM
If you want to use this format, DO NOT use your
return key unless you want to create a new line
for text. The return key will move all the text
down one line.Use the ARROW keys (to the lower right of your
keyboard) to move from line to line.Use the END and HOME keys (upper right of keyboard)
to move the cursor to the end or beginning (HOME)
of the line your cursor is on.To enter information, arrow down to the line you
want (or use your cursor and click on the line)
then hit the END key. This will place your cursor
where it needs to be so that you can begin typing.Remember to SAVE the document after you have made
changes. You can save the file to your hard drive
by selecting SAVE AS... and selecting your C:\
drive. Be sure to save it in a folder that will
be easy to find.Click on the version of the form you want to use:
Generic TEXT format (.txt file)
MS WORD format (.doc file)
